As OP, a quick follow up.
Xcode entered the indexing forever state when I built the system at a time when there were a few compile errors. I force quit Xcode a few times (a time consuming process because the infinte mode was causing Xcode to use massive amounts of VM [100s of Gb according to activity monitor]), restarted and rebuilt, always with the same result.
Finally, based on desperation, I restarted Xcode, snuck in as fast I could and fixed a couple compile errors before the VM got too high, and then force quit Xcode again.
On the next restart (an all future restarts) Xcode acted normally. My conclusion was that the infinite indexing was a side effect of the details of the specific compiler errors that I fixed. By the time I decided this I had made so many changes that I couldn't reconstruct the state that caused the forever indexing.
