What happened to "Edit All in Context"?

Did "Edit All in Context" go away, or am I not invoking the right magic gesture, or is Xcode being buggy?
I liked when "Edit All in Context" was a contextual menu and (I believe) in the Edit menu, because I could assign a hotkey.
I liked it less when it changed so I had to wait for that downward triangle to appear next to the symbol. That interaction is wrong in so many ways. It makes me wait, it requires precise placement, and it has poor discoverability. And I don't *think* there was a keyboard accelerator, but could be wrong.
But now I can't find it at all. When I select a local variable, all other occurrences of that variable get the dashed underline like they used to when I was in "Edit All in Context" mode. But editing the variable does not change all occurrences (nor would I want it to without me explicitly asking).
Running Xcode 4.5.2. What am I missing?
