Can't make over the air install work

I've done plenty of apps with a developer profile, a few UDIDs, and Distribute for Enterprise.
For the life of me, I can't get my latest to install. It runs fine with that profile on my phone, but when I try to install it over the air, I get
Unable to Download Application
"App" could not be downloaded at this time.
Done Retry
The log shows
SpringBoard[65] <Warning>: could not save thumbnail for downloading icon: image=(null)
path='/var/mobile/Library/SpringBoard/DownloadingIconImageCache/com....'
Googling turns up people having issues with keychain, others having issues with having ad-hoc and dev profiles (I only have a dev profile). None of those seem to be my issue.
I've deleted the failed download, rebooted. Don't know what else to do.
One note: the icon is always plain white; it doesn't show the app icon I get when I install via Xcode. Not sure at which point in the download it updates the icon, but it never gets that far (as suggested by the logging message.
