This also seems to happen 100% of the time if I turn Address Sanitizer on or off in the scheme settings and rebuild. Or if my Test action uses the sanitizer but Run doesn’t, and I first Run and then Test (or vice versa.) This triggers a rebuild, which then fails because the dylib is busted.
