Rich,
your problem is definitively a permission problem.
Either the folder containing the XTension or the folder
which stores the prefs is having the wrong permissions.
I had this kind of problem on a customers computer last
week, but related to the PS Import XTension.
I were not able to figure this out exactly, fact is I could only
get all XTensions to run for the user who installed Quark.
No chance for the admin, no chance for the second user.

Has anyone experienced a problem with Quark 6.5 CMS ext. I have 8 Macs
running OSX 10.6 all with Quark 6.5 Recently 2 of the Quark Versions
cannot launch unless the CMS ext is disabled. Nothing to my knowledge
has been installed. There has been monitor calibration being done but I
have not been able to link the issue to this. Any input would be
helpful.
