I have the same issue, but I remember having them working differently
in the past. It's highly possible in the ugly pipe from quartz to X
key events that I crossed wires. The Mode_switch switch is actually
quite hackish since Apple doesn't actually have a keysym for
Mode_switch.
Please still file the bug to keep me honest, and sorry for the
regression. You can grab Xquartz-1.3a1 and drop it in to have Alt in
the mean time.

Maybe your left and right Alt keys are the same? Run xev, place the
mouse cursor in the little window that pops up, and press and release
buttons. You will then see both the keycodes and the keysyms of the

Indeed... both Alt keys have keycode 69. Is this a configuration or
a hardware-thing? I have the older (white) Apple bluetooth keyboard.
