When you add an item or items to a project, you get a modal sheet with options that control how it will be added. In that sheet are two radio buttons:
Recursively create groups for any added folders
Create Folder References for any added folders
The first option creates a source group for each folder that you add. Files within that folder are then recursively added as new source file references. Any subfolders create a nested source group and its contents are recursively processed, ad infinitum.
You can always create new source groups manually using the New Group... command.
If you choose the second option, adding a folder will create a single source folder reference for the entire contents of the added folder. Files within that folder are not examined and are not added to the project as individual source references.
