I want to rename this method. In the .m file, I am double-clicking on the isUpToDate text so that it is highlighted entirely. I've tried highlighting lots of other code as a double-check, but nothing seems to cause the menu to become enabled.

Works for me, provided the method has been compiled. I added a method with that name to an existing class, selected it, and the Refactor... command was disabled. I built the project, selected the method name again, and Refactor... was enabled.
