Ideally it's the same way as any other program on Mac OS X. However it is only under Safari, which uses java 1.4.1 or later. Internet Explorer uses java 1.3.1, and the Accessibility support isn't in java 1.3.1 (I don't know about IE offhand).

Go to the System Preferences, go to Universal Access and enable access for assistive devices, and then go to Speech, and enable the speech level you'd want there (such as speak what's under the mouse).

If you have troubles with your java applet in Safari, please file a bug report under "Java (new bugs)". Thanks.
