Is your app Carbon or Cocoa? It sounds like which framework is in use by your app isn't installing its accessibility callbacks (part of the internal implementation of accessibility) at launch. Normally this happens automatically.

Carbon. Yeah, that's what it sounds like, and I couldn't figure out why it's happening.
