If you go to Quark's website they have some info on the subject. Their tech support was very helpful(believe it or not). They told me one way to get custom page sizes is to hack a ppd file. You can also use the "generic imagesetter" ppd but I kept running into trouble with color. The only way I was able to get custom page sizes was to hack a ppd file.
All I am doing is printing a PostScript file from Quark. I am not trying to send directly to a printer nor am I using desktop printers.
I am sure there are others on the list who know more and have different solutions but that's my 2 cents.
