Tero, I'd imagine coding this yourself could be a major undertaking, but then
I'm just a AppleScript Newbie. There are numerous tools/applications available
to work with PDF files. Check out www.pdfzone.com, www.pdfstore.com and
www.planetpdf.com for good resources.

Depending on your requirements and features needed, you'll spend between $150
and thousands of dollars for various solutions. I know of the following products
that may be of help...

ARTS PDF Workshop
ARTS Split and Merge Wizard
Appligent AppendPDF (also has a Pro Version)

There may be more that could do what you want. These are the ones I know of.
