Third-part osaxen is what Chris means by "other". And there aren't any
in OS
X that will do this currently, just in OS 7/8/9.

I can't verify that it offers cursor positioning in the dialog since I
don't have it. It does provide for extensively customizable dialogs and
is available for OS X.
