After any of these steps, did you try logging out and in? It seems to me
that sometimes that acts like "rebuilding the desktop" did in Classic
systems. For example, when I get a new beta build ("B") of an app and still
have the previous version ("A") OMM, if I remove the old one (A) from the
dock and drag the new one (B) there, then click the Dock icon (B), it's A
that opens up, with a second icon in the dock. This goes on happening until
I log out and back in. I have no idea what causes this, but it does seem
quite consistent.
