I have zero spam getting through (no false negatives). I catch the odd legit e-mail message in the spam folder (false positives), but not much. I just use the "does not intersect address book" filter rule and keep the address book up to date. So far it's the most effective thing I've tried. From time to time, particularly after a couple of drinks, I send something abusive to the 419 spammers (you know their mail addresses are valid), but otherwise it gets nuked after a quick inspection for legit mail.
