Try an FMP plug-in such as Events Plug-in <http://wmotion.com/events.html>. I imagine that this'll carry less overhead than searching FMP every <time period> for an event. I haven't used this plug-in but I hope it allows you to run scripts at a certain time based on the values in date / time fields. If this is true, you can then use an AppleScript from within FMP to send an email via "do shell script".

Having re-read your post, why don't you take a look at Lasso or a similar product? Lasso runs with MySQL as a back end and has an events database. It'll send emails too so one way or another you've got your bases covered.
