The copyright notes from my copies suggest 1999 (ASLG), 1996 (ASAG) and 1994
(ASFG). In spite of their age, I still find all useful from time to time.

With regard to your recently declared preference for hard copy, I'm not sure
about availability in that form.
(I hope my innocent mention of an AS reference doesn't spark off yet another
