What you want is for iTunes to be attachable. Notification events can then be sent to user-attached scripts whenever interesting things like track changes occur. See Folder Actions, Mail rules, AOL Instant Messenger events, [event-driven] GUI toolkits like Smile and Studio, etc. for examples.
