Hi Bob - this is the standard behavior for an application that was saved as a "script application" from Script Editor, with the "show startup screen" option checked.

I suggest opening the offending script and resaving it, either as a plain compiled script, or as a script application with the startup screen option unchecked.

I'm not sure why it would only appear some of the time. It seems like it should appear all of the time, unless the script app happens to already be launched, maybe?
