Years ago I used to open the file for reading. Grab the first chunk of it and look for the IPTC markers.
All the IPTC data starts with 1C 02
Next byte denoted the field name
78 = caption
7A = caption writer
69 = Headline
28 = Spec. Inst
50 = Byline
Next two bytes denoted the size of the info package.
so... 1C 02 78 00 19 = 28 bytes of caption information
I think some of the image data has been shuffled around since Photoshop 7 but this used to work for me and I didn't have to use an external reader to do it.
