You can prepare an ASCII text file with extension .fdf from which Acrobat will "import form data". The format of the file is published but it's pretty easy to get a prototype using "export form data" from a previously filled out form with the same fields in it.
I don't know if you need professional versions of Acrobat. I use Acrobat 5.1, the full version, on this 8500 running OS 9.1 and it fills out my US income tax forms from data on an Excel spreadsheet.
