Mask to input many data

I would like to use applescript to build a procedure to input data to a database table.
My idea is to have one mask only to input all the data for each record (in the sense of a database, not of applescript) in a single shot, e.g., name, surname, address, telephone (this is a single record). Then I would transfer the variables into which data have been placed into the text of an SQL query to insert these data  via " do shell script" into my mysql db.
Is there any procedure available to build this kind of mult-input masks?
