Don't remember seeing that but I have one of them this morning.
I have the explanation.
With 10.11, recent items aren't stored as they were before.
They are now stored in files like :
SSD 500:Users:<me>:Library:Application Support:com.apple.sharedfilelist:com.apple.LSSharedFileList.ApplicationRecentDocuments:com.apple.scripteditor2.sfl
It seems that in the change process, they introduced a bug.
The phantoms are the remembrance of sdef files which we opened and are no longer open.
You may easily check that.
Open a dictionary
Look the Recent Items menu contents.
You will see the entry displaying the opened sdef.
Close the dictionary window
Look at the Recent Items menu contents.
You will see the phantom entry.
