I just ran across this app:

I've never used it, but it is cheap ($12) and looks interesting. If it uses a
SQLite DB, then you could use Shane Stanley's SQLite DB Lib to move data
between it and your other apps.
Of particular note, is that it has a WYSIWYG form designer.
It is also available for $1 from https://bundlehunt.com, when you buy a bundle
for $7.
I did not see anything about scripting support, so it most likely does not
directly support it.

Thanks guys — at this point I don't have anything remotely as formal as
requirements — just sort of a nagging frustration that some of my favorite apps
aren't giving me exactly the features that I want. Right now I'm mainly fishing
for ideas, especially if others have tried anything along the lines I mentioned
that I could learn from, maybe first of all by helping me clarify just what I
do want to accomplish.
