OK, I tried connecting the "Hide Toolbar" menu item to the
First Responder instead of my window. The menu text still
doesn't toggle to "Show Toolbar".

Try connecting the action to the First Responder instead of your
window. That's what it says to do in the docs, and although I
can't think exactly of why this would make a difference, that's
how my apps are set up and it works fine.
