YOU have to click and hold the mouse button down for a second before
you start the drag. This is how it works for all carbon apps
(probably because it worked that way on Mac OS 9), I believe. I
wasn't aware that cocoa behaved differently. Seems like one of those
things that needs to be addressed one way or another.

If I drag on the icon in the title of a Finder window, the window
doesn't move, but there's no drag icon, and no drag highlighting if
I drag the mouse around.

I'm afraid I don't understand what you mean. You can drag a finder
window's proxy it to move the folder represented by the window to
another location, so in that regard, the seem like proper proxies
for the folder to me.
