No, mostly they used the public NSStatusItem API... The only thing I
know of that uses a transparent window that sits above the menubar is
Classic Menu or whatever it's called that places its own apple menu over
the standard one, and it's so obviously an evil hack it's amusing.

well yes, but as has been discussed before, that's not always feasible.
For example, what if you want to provide something that has similar
functionality to shipped NSMenuExtras? Why doesn't Apple just use
application dock menus instead? Because it wouldn't work, they're meant
for different things. But that's offtopic... :-)
