Is it simply a space issue? I've noticed that with some apps the two
different sets of menus (the app's menus on the left and the menu
extras on the right) overlap and that causes some issues. (unless of
course you've got a nice 22" cinema display, or a TiBook)
I believe someone from apple already posted about this a few weeks
back. Apple considers that area "reserved" so it can add
hardware-dependent information into that area. Notice that all the
applied supplied menu extras (except for the sample Script Menu) are
hardware indicators (Battery, Sound, Time, Displays, Airport, etc.).

You can't. Those are off-limits to third party developers. Some of
the older hacks used transparent windows which sat above the menu bar
but I'd strongly suggest using application dock menus instead.
