The real reason being though rather the backward compatibility with OS9
users. I've taught newbies NeXTStep quite often; there were things they had
problems to grasp, but creating folders as a sideeffect of saving was *NEVER*
an issue.

Actually, even for one level it is much easier just to write
"folder/filename" and press Enter (twice) than doing it the clumsy way
through an extra button and dialogue.
That's not really important though. Both ways should ba available so as OS9
or windoze users can continue using the way they are used to. Vast majority
of them would learn the better way eventually of course, but it would not be
reasonable to force them to it.
(Much more irrational though is the current OSX behaviour which prevents the
easier way to be used :(((( )
Incidentally, I've just recalled another grave problem of save panels -- see
my message in Cocoa list with subj. "UI improvements (was: cocoa-dev digest,
Vol 1 #581 - 19 msgs)".
