since the question proved to be suprisingly tricky, I've tried to compile a (reasonably) complete sum, with a very kind help of Ali Ozer (nevertheless, any potential errors in this text are completely mine).

Sorry to be late to the party here, but I already did this a while back (again thanks to Ali):

I daresay it contains almost precisely the same I've written, but in much more details. The only things mmalc does not have whilst I do are the dealloc considerations, and the multithreaded accessor with the compare (not to copy always for a price of an extra lock/unlock pair), and it is pretty special thing anyway.

Whoever still does not completely understand the accessor issue, I highly recommend the article.

Scott, mmalc: I think it would be better if mmalc slightly updated his article than putting mine beside -- mine says almost the same, but in *MUCH* less comprehensible way (well, first, mmalc is better than me; besides, he wrote an article, whilst I just a short e-mail). Nevertheless,
it's completely up to you; if you feel like, do put mine there freely.
