Just for the record, I barely caught the same mistake in my own book in time
to fix it. And I had that opportunity only because I insisted that my
publisher let me see the final proofs before they went to the printer. There
were also some problems with straight quotes in code being converted
automatically to curly quotes -- I caught (almost) all of those in time,
too.
I'm not particularly surprised to discover that it's difficult for
publishers to remember to turn off the autocorrect feature in MS Word or
Quark or whatever when preparing technical texts for printing. Everybody is
very tired by that point -- authors, editors, copy editors, publishers --
everybody. And I gather it's not uncommon for this step to happen AFTER the
author's last opportunity to review the product.
If you're like me and you insist on proofing your own work product, be sure
to get a good night's sleep beforehand. Proofing several hundred pages of
code for em dashes and curly quotes where they don't belong (and straight
quotes that are supposed to be curly quotes, in the second round of
proofing) is not easy, especially under deadline.
