Check you document icns file has all of the icons filled, I had a
problem where my icon would not show up until I re-edited the icns file
and add all of the other size not just the thumbnail one. Could of been
just a coincident though.

I had moved it to the Applications folder. Just to be on the safe
side,
I've also logged out and back in. I later rebooted. Just now, I also
removed the LS* files in ~/Library/Preferences and rebooted again. No
change.
