No, no, no.
You're trying to reason from first principles about how to design a
human interface, which *doesn't work*.
You have to design a number of prototypes, and then do large amounts
of user testing.
That's what Apple did to come up with the Human Interface Guidelines.
They found that no, even if the user chooses an option, the default
should *not* always be to confirm the option. Instead, if the option
could irretrievably destroy data, the default should *not* destroy
data.
Of course, with the Human Interface Guidelines, most Macintosh
developers get the benefit of the many tens of millions of dollars
Apple has spent on prototyping and user testing over the years "for
free" just by adhering to them.
