I have no real ideas here apart from to verify that all the links are
completely correct and are relative to the file containing the link.

Specify the path up to the Help folder. Eg

<meta name="AppleIcon" content="My Help Folder/icon.gif">

It has to be a 16x16 image, from what I can remember from
Anguish/Buck/Yacktman.
