I found out that Apple knows about this problem:

Basically, turn off the Equalizer, Sound Enhancer and Sound Check to avoid the problem of the volume fluctuating.

I listen to MP3s while programming, and without it, I don't get much work done.

iTunes 4 for some reason varies the volume when playing MP3. The sound gets louder, then softer, then louder again. It's super annoying.

Can someone on the iTunes team post a fix? This is really annoying especially since iTunes 1,2,3 sounded great.

I've got all the prefs turned off for Sound Enhancer and Sound Check. I listen with headphones, so I can't say if it's noticeable through speakers.
