I had these symptoms, and it turned out to be something in my html
files that Panther's Help Viewer didn't like -- specifically, dubious
syntax for a content-type META tag in my index.html. The malformed
construct didn't bother any web browsers I tried, and even Help Viewer
itself would display the help properly if I opened it from Help
Viewer's File menu. The breakage seemed to be in somehow preventing
Help Viewer from registering the existence of my help book and adding
it to the library.
I tracked this down by experimenting with working vs. non-working Help
files. Be aware that the cache in ~/Library/Caches/com.apple.helpui
can affect your experiments; I found it necessary to clear that out for
each trial.
