You didn't mention whether or not you used the "Icon Composer"
application to create an .icns file from your .tif. The application is
located in /Developer/Applications/Utilities/ and includes help if you
need it. It is quite easy - create a new file with the app, then drag
and drop your .tif onto the squares you will see in the interface.
Then save.
I also noticed (years ago, might be different now) that when saving
from Photoshop I have to specify "none" or "LZW" for compression - the
other options did not work properly.
