Does anyone know if the image-selection window used by iSight when
choosing a new avatar or by the Address Book application to assign a
new picture is available outside Apple? My searching has not been
fruitful...

The functionality is provided by an undocumented framework inside the standard AddressBook.framework and, being undocumented, is subject to change without notice. That said, if you're building something for personal use you can link into this at your own risk. I wrote some notes about this back in January and posted them to this list. See the link below.
