I don't know if this is related, but I've noticed that the Apple menu extras will sometimes not respond to menu-selections right after the computer wakes up - specifically, the "Connect" item in the PPP extra. The menu will drop down (sometimes, sometimes the click will "miss" as Tavis is saying), but then when I select an item, it won't work. I don't know if this is because of some bug in the menu handling code in OSX not working until a second or two after wake-up, or whether it's with the action the menu item triggers, or with the specific menu extra class.
