You should probably direct this to the Xcode-Users mailing list. It's something that pops up every now and then. I think it's a bug in Xcode that they haven't managed to reproduce yet.

I discovered in the Contextual Menu an 'Update' command that I can find no documentation about. Applying this first before a Build and Run seems to work fine, but it's frustrating to have to select this each time and I can't find it on any menubar, with a shortcut key to speed up the process!

The "Update" context menu items is grouped with other SCM items and is probably not directly related to your problem [1]. It might be that triggering a SCM update will either flush your changes to disk, or simply allow for enough time to pass to have your changes propagate to the build system before you build.

[1] I can't find this menu item in the Xcode PDF, so it's difficult to know exactly what it does. I don't dare try it...
