I have been unable to figure out how to make this work — the documentation is less than clear in this respect. It seems to say that if the custom view is a control it will be displayed in regular or small size but this does not actually seem to happen.

Does anyone have any suggestion on how to make this happen?

It's a long-running documentation hole. If your toolbar item is a custom view, then it must override -setControlSize: in order to be notified when the toolbar is changing sizes.
