You are treading on dangerous ground here: Pantone CMYK equivalents are
device specific, and do not convert similarly through photographic
conversions (Perceptual intent) to how they are processed through vector
image conversions (Relative Colorimetric conversion). You would be much
better off getting Lab values for the Pantone color, unless you have a
guarantee that the output is going to be a SWOP standards press, and you can
do all the color conversions on your own computer. If that is all you need,
you can trick Photoshop into giving you the Pantone numbers you need, or
simply supplying the swatch you want as the foreground color.
