A good description overall but you left out where you are converting
to the Stylus Photo EX color. When you look at the image in PS and
see the banding, is that the original image or after conversion? The
answer to that will tell us whether the Epson profile or the PhotoCD
(or your film, I suppose) is causing the banding.
Second, and this one is a current image I'm
working on, I have a shot of a plate of pasta with red sauce on it.
Looks great on the monitor, pretty darn close to the film, but a print
renders the highlight side of the tomato sauce wildly red, unnaturally
pumped up. In both cases, I wind up spending a lot of time going in and
doing some kind of brute force retouching to overcome the problem.
This sounds like the profile you are using for the Epson is no great.
Are you using a canned or custom profile?
You may have two separate problems here or they may both be related
to the Epson profile...
