I'm using Photoshop 6.0 on a G4, with AdobeRGB as my RGB working space, and
Phaser 860 Photo (which is the profile of the Tektronix printer I mainly
use) as my CMYK space,
If I
just create a blank white page, that measures pure white, use Convert to
Profile to go from RGB to CMYK, I get the dots every time.

The effect is due to non-standard white point calculation. The effect appears / disappears depending on the combination of CMM and make / model of profile in the profile chain.

To troubleshoot your profile chain, first replace the profile for your Tetronix printer. Use a profile made with ProfileMaker 315, or other ICC profiler with standard white point definition.

IMO you should not be targetting work for your studio printer except for full gamut presentation prints. Before you have a friend build you a new profile, set up your printer with consumables that maximize its color space and check that the printer is stable from print to print or proof to proof.

Hope this helps.
