At which stage, just before starting a new profile? Can an existing profile with this problem be corrected somehow in edit, or should I restart from scratch?

Yes, some call it SCUM dot. I have not seen this with Gretag CMYK profiles.
ColorBlind changed its core engine when rendering data RGB to XYZ to L*a*b*.
It was discovered that some equations yielded dots and others did not with
various CMMs. We did find that the dots were lessened when trashing
Photoshop's and ColorBlind's preferences.
