When you create your adjustment layer, there is an option for "group with
previous". That will restrict the effect to the layer directly beneath it.
You can also option-click in between the adjustment layer and layer you
want to effect to group those layers together. Additionally, you can just
paint on the adjustment layer mask where you don't want the effect to
occur, or fill the whole mask with black ad paint in with white where you
do want it to occur, or...
