Use the ColorSync Utility. Click on the Devices button and you'll be
able to select your printer model by turning down the "Printers" expand
triangle. Then turn down the expand triangle for your model of printer,
select the media setting you will be using, and then set the Current
Profile drop-down menu to the custom profile.
The rendering intent used is the default rendering intent specified by
the profile. As of yet, there is no rendering intent control that I've
seen in OS X printer drivers thus far. One step forward (you can't use
custom profiles with (at least Epson's) OS 9 printer drivers), and one
step backward (you can specify a rendering intent with most any
manufacturer's OS 9 drivers).
