The only way to use grayscale profiles in InDesign is to
place PDF files with embedded grayscale profiles.

And that's kindof scary considering what happens if you ask InDesign to
do conversions on RGB or CMYK PDF's (or native AI files). Bad things...

I think for the time being it will be necessary to place output ready
grayscale images into page layout applications (QuarkXPress doesn't do
grayscale management either). And when asking InDesign to do color
management, use known reliable file formats: PSD, JPEG, and TIFF. I
think PICT is also supported. The only way I've been able to get
reliable conversion of placed PDF and AI objects in InDesign is to
export as a PDF; but when it goes through the print path, I've seen
major discrepancies in conversions compared to the same data placed as
TIFF or PSD.

I used Lab conversions (of grayscale images) and obtained good results. InDesign do support Lab images and manage them correctly. I didn't check with measuring instruments but outputs looked O.K.
