"This isn't a problem unique to Monaco. In my experience the DTP41 is
rather more fussy over the spacers between the patches than the
manufacturer would like to believe. Putting down black or
white patch spacers is not sufficient in itself, unless the
layout of the patches is very carefully controlled so as to
avoid certain colors being adjacent. Randomizing the patch
layout may raise your chances of running into this sort of problem.
Another "gotcha" is cutting the strip too short at the front."

It is true that read errors with the DTP41 are not unique to Monaco, but I
have experienced read errors that have been caused by the patch generation
in Monaco Profiler. I'm talking about the not-so-careful placement of
patches that makes black line generation necessary, and - the inappropriate
placement of black lines between darker/denser patches. This is something
that has occurred on a variety of media types including standard
photographic substrates, not just backlit stocks. It also seems to occur
more frequently on custom page sizes.

That being said, there are different ways to address this issue. One method
would be to save out a tif of your patch set and manually place or delete
black lines from the row(s) that are causing the error (in Photoshop) - then
reprint the patches and try reading them again. Another method would be to
change the default color space in your OS and generate new patches. Profiler
references your default system color space when it 'decides' where to place
black lines. Depending on which color space you choose you might see a
variation in black line placement.
