As I described earlier today, when you turn on the paper white simulation,
it only affects the image, not the UI. You'll seriously fool your brain
unless you work in full screen mode to hide all the white UI elements that
don't undergo the paper white simulation. This is one reason why turning on
the simulations are known by many as the 'make my image look ugly button'.
Even with dual display, you have to be careful about the differences in the
whites here. Unfortunately, according to Adobe, they can't alter the white
of UI elements (that's an operating system issue).

Just using the black simulation isn't as useful as the full Monty. And BPC
is on unless you use the Black Ink check box which just turns it off for the
soft proof (the default is RelCol with BPC).

It's on by default. To better simulate the black, you're turning it off when
you use this check box (Simulate Black Ink). When this isn't being used,
Photoshop assumes 0/0/0 is a black hole which it isn't of course.
Yes I'm using CS2 and CS3. They behave the same way.
